What will I get if I register with the Index
When you register with the Index you will receive regular information about the range of events, activities and support available for children and young people with disabilities or additional needs and their families. Registering will also provide you with an opportunity to help inform the development of new services in Cardiff.
Declaration
By completing this registration form you are providing your consent for your personal data to be used to add you onto The Index.
Information about events, activities and support will be sent to you via the email address you provide on this registration form.
We will process your disability data for the purpose of understanding what events, activities and support are available to those registered on The Index.
We will process your ethnicity for the purpose of aligning with fellow council services, and to support future planning of the Index. We will collect this information with your consent, and it is not mandatory for you to provide this information.
The Index provides support to young people ages 0-25. When you/your child turns 25 your/their personal data will be removed from The Index unless you withdraw your consent prior to this. The age 25 is stated, as we are looking to increase our upper age limit to help support those in the transition period of 18-25 also.
You can withdraw your consent at any time by contacting theindex@cardiff.gov.uk or by calling the Family Gateway Team on 03000 133 133. If you withdraw your consent, we will remove your personal data from The Index and you will no longer receive information from us.
The personal data you provide on this registration form will be processed in line with current data protection legislation.
